| When will EnvisionRx Plus plans be available? |
| EnvisionRx Plus offers two plans - EnvisionRx Plus Standard and EnvisionRx Plus Gold - which are available in all 50 states and the District of Columbia beginning January 1, 2007. Open enrollment begins on November 15th, 2007. |

| Is it really necessary to have prescription coverage? |
| The costs for prescription medications continue to grow each year.
Enrolling in a Medicare approved prescription drug plan helps you
to reduce your out-of-pocket costs for prescription drugs.
EnvisionRx Plus uses a prescription drug formulary to lower your
costs and ensure access to quality, safe, cost effective medications. |

| What is a formulary? |
| A formulary is a list of covered drugs selected by EnvisionRx Plus
in consultation with a team of health care providers, which
represents the prescription therapies believed to be a necessary part
of a quality treatment program. EnvisionRx Plus will generally
cover the drugs listed in our formulary as long as the drug is
medically necessary, the prescription is filled at an EnvisionRx
Plus network pharmacy, and other plan rules are followed. For

| What if a medication I’m already taking is not included on the preferred drug list? |
| To ease your transition into an EnvisionRx Plus plan, allowances
will be made during the first 90 days of coverage to allow you time
for you to continue on your current medication, while you and your
physician discuss your treatment options. |

| Which pharmacies can I use to get covered prescriptions filled? |
| There are more than 57,000 pharmacies that participate in the
EnvisionRx Plus national network. This includes pharmacies like
Costco, CVS, Longs, Rite Aid, Walgreens, and a large choice of
independent pharmacies. |

| What is the advantage of using Costco instead of other pharmacies? |
| You can get preferred generic drugs at no out-of-pocket cost, whether
you use a 30 day or 90 day supply – even through the “donut–
hole” - at Costco retail pharmacies. Additionally, your copay for
brand drugs will be lower. Costco membership is not required. |

| Can I also get mail order prescriptions from Costco? |
| Yes. If you take daily medications, covered prescription drugs are
delivered to your home by Costco Pharmacy. |

| What is my monthly premium for the Standard Plan or the Gold Plan? |
| The premium varies from state-to-state. Contact a Customer
Service Representative for more information or visit our website. |

| How do I pay plan premiums? |
| You can have your monthly plan premiums deducted from your
Social Security checks (just like your Part B premium) or you can
pay your premium to EnvisionRx Plus directly. Premium payments
may be made by credit card, check, or electronic funds transfer. |

| How do I enroll? |
| You can obtain an informational packet and enrollment form by
contacting a Customer Service Representative or you can enroll
on-line by visiting our website. Open enrollment for 2008 begins
on November 15th, 2007. |

| What is a medication therapy management (MTM) program? |
A Medication Therapy Management (MTM) Program is a benefit that EnvisionRx Plus offers to qualifying members. The goal of EnvisionRx Plus’ MTM program is to help raise awareness of the importance of your current medication regimen in managing your disease states. All members who meet the following criteria will be enrolled in the EnvisionRx Plus MTM program. - you have the following disease states: High Cholesterol, Diabetes, and Hypertension
- Be on at least 5 different covered Part D drugs for these disease states
- Average $1,000/quarter on your total drug spend
It is recommended that you take full advantage of this covered benefit if you are selected. You may opt out by calling Member Services at 1-866-250-2005 24 hours a day, 7 days a week. TTY users should call 1-866-763-9630. |
